COLLABORATION KIT FORM

Please fill in the form below summarising your study with specific mention on how you wish to use QGel. The QGel team meets weekly to evaluate and select the projects submitted. To participate in this project, the QGel Kit is offered FREE OF CHARGE but we kindly ask for a contribution to shipping and project handling fees.

If your project is selected, QGel will follow-up with you promptly after our next team meeting. You will receive an invoice for shipping and handling costs and, upon payment receipt, your QGel trial kit will be delivered within a few days by express delivery.
